 AOE/COE (Statement Taking) Investigation:

This investigation involves an in depth inquiry into the claimant's medical, employment, and personal history.  The interview also covers the claimant's hobbies, current treating physicians, and extent of the present disability.  The most detailed portion of the investigation involves questioning the claimant and witnesses with respect to the physical aspects of the job duties and the exact details and circumstances surrounding the alleged injury.  

Subrogation - Third Party Investigations:

The main purpose of a Subrogation or 3rd. Party investigation is determine if a party (3rd. Party) other than the employer or the insurance company is liable for the injured workers' condition.  The employer has the right of recovery or Subrogation against the negligent third party.  A subrogation investigation covers issues also covered during an AOE/COE investigation.  However, the investigation report focuses on the identification of all parties (ie. subcontractors, machinery manufactures, inspectors, equipment renters, leasers, suppliers, and drivers for off sight traffic accidents) potentially liable for the injury.  The investigator also identifies the insurance carriers for potential subrogation.  It is critical that the investigator and claims examiner raise questions when the  possibility for subrogation is discovered during the investigation process.

Subrosa (Surveillance) Investigation:

This investigation typically involves 8 - 12 hours of on-sight surveillance at the claimant's reported address.  Our policy is to spread the hours over two days to increase the chances of observing the claimant active.  Week day surveillance typically began at 6:00 a.m. while weekend assignments began at 7:00 a.m. unless otherwise instructed.  All investigator's are instructed to verify that the claimant is inside the residence by 10:00 a.m. if no activity has been observed.  A neighborhood level Activity Check is also conducted to increase the likelihood of obtaining video evidence. If the claimant is active, our policy is to stay on the subject until he/she arrives back at the residence.

Activity Check

This investigation involves a neighborhood level inquiry into the claimant's activities, habits and hobbies.  Its primary purpose is to assist in determining if a surveillance is needed.  Various subterfuges are used in obtaining information about the claimant's activities.  All investigator's are also instructed to observe the claimant's residence for at least 2 - 4 hours depending upon the case.

If the claimant becomes active during the Activity Check, our policy is to stay on the subject and obtain documented video evidence.  The case is then billed at the regular hourly rate of a surveillance investigation.

Documented Video Evidence

Our investigator’s roll film with high 8 or digital 8 format.  All surveillance cameras display simultaneous date and time and have both auto and manual focus.  Each investigator is also provided with a small hidden camera to in order to discretely roll film of our subjects in areas where they are unaware they could be filmed.

Court

If an investigator's testimony and/or documented video evidence is needed in court, our policy is to bill at our standard hourly rate for each investigator and a minimum of 4 hours.
